Job Description:**Job Title:** Case Manager

**Reports To:** Inpatient Department Manager

**Job Summary:**

The Case Manager is responsible for ensuring the provision of optimal patient care through effective coordination with physicians, nurses, hospital administration, and external healthcare providers. This position involves the proactive identification and resolution of deficiencies in patient care plans, in collaboration with relevant stakeholders, to enhance patient safety, improve care quality, and optimize resource utilization.

**Key Responsibilities:**

* Coordinate patient care activities amongst physicians, nurses, and other pertinent departments to ensure seamless integration of services.
* Identify and analyze discrepancies in existing care plans and implement corrective actions utilizing available resources.
* Conduct routine rounds within nursing units to assess new patient admissions and monitor ongoing cases.
* Facilitate timely discharge for patients upon completion of their prescribed care plans.
* Review requests for investigations, procedures, and consultations, ensuring prompt and efficient service delivery.
* Maintain meticulous documentation and ensure strict adherence to established care plans.
* Develop and maintain a daily registry of “high-priority patients” for focused monitoring and intervention.
* Monitor intricate patient cases requiring specialized attention, including but not limited to, abnormal laboratory results, prolonged lengths of stay, changes in level of care, discrepancies between unit assignment and patient needs, re-admissions, transfusions, and other complex clinical scenarios.
* Collect and analyze data to inform and support evidence-based clinical decision-making.
* Submit a daily case management report to the Medical Director, summarizing key patient management activities and observations.
* Prepare a comprehensive monthly report encompassing occupancy rates, bed turnover rates, patient distribution, sources of admission, average length of stay, mortality rates, surgical classifications, service utilization patterns, and recommendations for process improvement.

Skills:

Education:

  • Bachelor’s degree in Pharmacy.
  • 0–2 years of experience in Case Management or a similar field.
  • Awareness of Clinical Practice Guidelines (CPGs).
  • Preferred: Diploma or Master’s in Hospital Management, Total Quality Management, or MBA.
